Commonwealth Care Link

The Commonwealth Carelink Centre is a convenient way to access information about what is available for people who are frail aged, have a disability and their carers. CareWest continues to conduct a promotional outreach program across the region to ensure equal access to the service. The Centre is a single point of contact for all Australian to access information about community, disability and aged care services.

Commonwealth Carer Respite Centre

The Carer Respite Centre is a part of a National network of Carer Respite Centres which provide assistance to carers of people who are frail aged, who have a disability or who have a chronic illness.The program is funded by the Commonwealth Government under the National Respite for Carers Program. The CWCCRC and CCC provide quality service that is ably administered by a dedicated staff.

Community Options Program

This program funded by the Depart of Ageing Disability and Home Care provides individual case management for people in the Home and Community Care target group residing in parts of Central west of New South wales who are at risk of inappropriate or premature placement in residential care, have complex needs and have a high need for ongoing monitoring.

ComPacks

ComPacks is a case management model of short term packaged care for patients post discharge from participating hospitals. It is designed to for people who are able to return home safely and are in need of at least two HACC services to assist them in their recovery.

Older Parent Carers Program

The Support Coordination Program is designed to complement existing      services and to target the ‘hidden’ carer.
The Support Coordination Program will work with Older Parent Carers to  determine what supports and assistance may be of greatest benefit  to ensure that your son/daughter  continues to receive  care and opportunities to live fulfilled lives. Contact 02 63912 400

 Community Aged Care Packages

The Community Aged Care packages offer a range of flexible services such as social support, personal care needs assistance, domestic assistance, shopping and personal business, business assistance, medication assistance, and assistance with other needs as determined by individual clients.

Aged Care Services

CareWest Aged Care Services provides respite and support to carers of people with dementia living in the local government areas aof orange, Cabonne,Weddin, and Cowra. CareWest DRS is also an approved provider for Veterans Home Care.

Child & Family Services

The Child & FAamily Services is funded through the Department of Community Services under a state funding initiative of Supporting Children with Additional Needs (SCAN). ECASS provides funding to children with additional needs in State funded children services.The criteria of funding are: Disability, Challenging Behaviour, Culturally and Linguistically Diverse (CALD) and Aboriginal and Torres Strait Islander (ATSI).

Transition to School

The Transition to School Project is funded through FNSW and is a twelve month pilot focussing on improving collaboration between agencies,families and communities in the provision of services that support entry to school. Children will be supported in becoming healthy and effective learners through a positive transition/interagency program. The Pilot areas are Cowra,Parkes and Bathurst.Contact Ellen Fisher Project Officer.

Disability Services

Carewest Disability service incorporates Post School Options, Transition to Work, Community Participation and Interchange Respite Care .The Programs provide valuable time out for carers and positive outcomes for services users.The case management service is provided across the Central West. All other services are based in Orange

Community Participation/Transition to Work

These programs aim to provide and facilitate activities for young adults with a disability who have completed year 12. Participants further develop skills acquired at school including personal, social and daily living skills, community participation, independence and self reliance through the associated Life Skills Program.

EarlyStart Diagnosis Support

EarlyStart Diagnosisi support Services is an innovative program that provides short term support for specifically for parents and families whose child has recently been idagnosed with a disability and/or are in the process of obtaining a diagnosis.

The program works directly with families, providing individually tailored support that recognises the strenghts of both the family and child in understanding the challenges, both prior to and following a diagnosis.

Home Modifications and Maintenance Service

The service specialises in home modifications, maintenance, adjustments and advice on building related matters. You are eligible if you are:-A frail aged person or a person with a disability who requires support and assistance or a person who has to care for an adult or a child with a disability; and you are a home owner or, in certain circumstances a private renter.
The aim of these services is to assist people to remain in their own home rather than having to move into a nursing home or hostel accommodation.

Community Transport

Community Transport is a scheme that provides safe, comfortable transport for disabled, aged and disadvantaged people and their carers.  The aim of the scheme is to enable people to remain living in their own homes and at the same time maintain their various activities in the local community.
Being partially Government funded, an initial registration process applies. Door to door transport is available for any purpose including day care, shopping and hospital or medical appointments.

CareWest Head office houses the administration section, finance and accounting,payroll and the Chief Executive Officer- Tim Curran. The office is open from 8.30a until 5pm five days a week.
